Please use the Transcript Request form to request a graduation verification, a current transcript for college, or for employment applications. Unofficial transcripts for current students may be requested at the school they attend.

All official transcript requests should be delivered to the Antioch Unified School District office located at 510 G Street, Antioch, CA 94509, along with a copy of your photo ID. The reception desk is open 7:30am to 4:30pm Monday-Friday, except for holidays. You may also email the form to us, once you complete it at Transcripts@AntiochSchools.net, along with a copy of your photo ID.
If you would like your transcripts mailed, you must include a stamped, addressed envelope for each transcript needed. A transcript is only official as long as it remains in the sealed envelope.
