As part of our ongoing commitment to supporting all students and families, Antioch Unified School District is asking families to complete a brief Mid-Year Housing Questionnaire.
This questionnaire helps us better understand whether a student’s housing situation may have changed during the 2025-2026 school year. Changes such as moving, staying with others temporarily, or experiencing housing instability can impact a student’s educational experience. By sharing this information, families help ensure students receive appropriate support and services available through the school district.

Completing the questionnaire is confidential.
The information is used only to support students and families.
Completing the form will not affect enrollment or immigration status in any way.
We understand that housing situations can change unexpectedly, and our goal is to partner with families to provide stability and assistance when needed.
